complete, and mail the appropriate form.
The forms will cover up to four students in a
family.
Please
be patient while the packet is opening (do not maximize
or use your mouse until it appears the packet has loaded on
your screen). Windows 7 users may need to refresh the pdf
window that opens. After the packet opens, click File,
Print, print All, and click OK.
If you have difficulty, right-click the packet above,
left-click Save Target As, save to your computer, then open
it from your computer.
